For First-time Applicants
Complete this questionnaire to order a customized Application Kit that includes the OMRI Standards and Policy Manuals, application forms, and checklist(s) that apply to your product. When we receive your completed questionnaire and $39 payment, we will send you both a paper copy and instructions on how to download the same materials from our website.

If you don't want to fill out the pre-application online, you can download and print a paper copy. Just fill out the form and mail or fax it to OMRI along with your payment. You can find our fax number and address right on the form.
